A condo or a new build with a concrete slab and radiant heat, where the design calls for herringbone. This is the specification most likely to be quoted wrong, because the pattern is chosen before the substrate is understood.

The part of a hardwood job that costs money is rarely the part in the quote. These are the failures that produce a second invoice.
Voids the mill warranty outright, and a floating herringbone field telegraphs every slab imperfection as hollow spots underfoot.
Avoided by: Full-spread glue-down in the warranted adhesive. There is no shortcut here.
Bond failure months later, under a finished floor, with the furniture in. The remedy is a full tear-out.
Avoided by: Testing the slab and buying the adhesive to the reading.
The pattern runs visibly out of square against the view the room is actually read from. It cannot be corrected without lifting the floor.
Avoided by: Setting the centre line to the dominant sightline and dry-laying it before glue.
